Comments for the student: Sophia came well-prepared and provided thoughtful and detailed answers to all of the questions asked. She provided stories and examples from her work, school, and extracurricular activities to support her responses. Sophia was very personable and comfortable in making the interview conversational, and was authentic and genuine in her answers. The one piece of feedback I had for future interviews is for Sophia not to shy away from speaking more directly and confidently; she has a great set of both technical and soft skills and I would encourage her to vocalize them more in conversations with hiring managers as she begins to interview for internships and future roles. Overall great job and I enjoyed getting to know Sophia!

If you are in direct contact with you interviewer, it is often considered a best practice to send a brief thank-you note following the interview. Within that note, you can share your interview feedback link.
Here is some sample language you can use within your thank-you note, which we encourage you to revise as you feel is most appropriate:
As I am new to the interviewing process and always looking to improve, I'd like to ask if you would take a minute to give me some (anonymous) interview feedback.
The following link [insert your interview feedback URL] contains just five general questions about my interview performance and is designed to be completed in under a minute.
